Once a patient is diagnosed with periodontal disease, it is important to be seen for a periodontal maintenance cleaning every 3-4 months.

Many of these patients currently wear dentures they have had for several years, if not decades. They most likely received their dentures before their cognitive changes began. All denture wearers develop muscle memory and a deep familiarity with their current prosthetics.

You may not realize it, but facial muscles and the tongue are constantly involved in helping to stabilize and keep dentures in place, especially the lower denture. This becomes evident after someone suffers a stroke. Before the stroke, there were no issues with how the dentures fit or functioned. Then, after the stroke, if facial paralysis occurs, the dentures may suddenly become loose or no longer work as they did before. Nothing changed about the dentures or the underlying bone, but the use of facial muscles and the tongue was compromised by the stroke, causing the dentures to malfunction or feel loose.

When a patient with dementia loses their dentures, the success rate of that patient being able to adapt to and function with new dentures is very low. And that’s if new dentures can even be made! Fabricating new dentures requires a significant level of cooperation and the ability to follow commands, which is not possible for many patients suffering from Alzheimer’s or dementia. The usual scenario when dentures are lost with a patient who has dementia is as follows:

• The family is upset with the caregivers or the facility about how the dentures were lost.
• The diet must be reduced to softer foods.
• The patient may lose weight from having no teeth or disliking pureed foods.
• There is a decline in overall health due to weight loss and lack of proper nutrition.
• Difficulty arises in finding a dentist, like Dr. Camille Dixon in Pompano Beach, FL, who will come to the facility or home, as it can be very challenging for the patient to leave their familiar environment for appointments.
• The patient is often without teeth for 4 to 6 weeks.
• Multiple appointments are needed to make new dentures—if the patient even cooperates for impressions and bite registrations.

• If new dentures can be made, the patient often cannot get used to the new teeth because they haven’t had them for several months and lack the mental capacity to acclimatize to the new dentures.
• The new dentures are frequently lost within the first six months.
